Monolaurin acts on the envelope, not on the virus in hiding

Two studies in the Journal of Fish Diseases reported that glycerol monolaurate disrupts the viral envelope and suppresses replication. The virus tested was cyprinid herpesvirus 2, which infects carp, not human HSV.

In vitro

Nearly all of this work comes from test tubes and animal models. It describes a mechanism, not a clinical outcome in people. PubMed indexes no clinical trial of monolaurin for herpes.

The mechanism

Three steps, and one clear limit

Contact with the envelope

HSV keeps its genetic material inside a layer of lipids and glycoproteins. Glycerol monolaurate is lipid-like, so it has an affinity for that layer and inserts itself into the bilayer.

Membrane destabilization

Packing of the bilayer loosens until the envelope loses integrity and breaks apart. Without it, the particle cannot bind to cell receptors.

Replication interference

A second action inside the cell may disrupt the viral multiplication cycle. This was reported in carp cells against a fish herpesvirus, so it has not been shown for HSV in people.

Limit

Latent virus is out of reach

No compound or drug eradicates virus lying dormant in the nerve ganglia. Envelope disruption applies to active particles only.
